Description
Running down the posterior thigh, the sciatic nerve descends deep to the hamstring muscles before dividing at the superior angle of the popliteal fossa into the tibial nerve medially and the common fibular (peroneal) nerve laterally. Within the popliteal fossa, the tibial nerve tracks inferiorly in close association with the popliteal vessels, then continues deep to the gastrocnemius and soleus as it approaches the posterior compartment of the leg. Superficial landmarks include the medial and lateral heads of gastrocnemius framing the fossa and the Achilles (calcaneal) tendon forming distally. Relationships matter here. Clinically, this posterior view clarifies the neurovascular layering that guides safe dissection and operative exposure in the popliteal region, where the tibial nerve lies superficial to the popliteal vein, and the vein lies superficial to the popliteal artery. That arrangement becomes relevant during posterior approaches to popliteal artery aneurysm repair or thrombectomy, and it also explains why iatrogenic nerve injury can occur when working deep in the fossa. The proximal course from sciatic to tibial also anchors teaching around radicular pain patterns versus peripheral neuropathy, and it helps localize lesions that spare the common fibular distribution but weaken plantarflexion and toe flexion.
