Description
Arising from contributions of the tibial and common fibular (peroneal) nerves, the sural nerve descends in the posterior-lateral compartment of the leg, tracking superficially along the calf toward the ankle. From a lateral perspective, it runs inferior to the lateral head of gastrocnemius and alongside the Achilles (calcaneal) tendon, then curves posterior to the lateral malleolus to continue as the lateral dorsal cutaneous nerve of the foot. The distal tibia and fibula frame the ankle mortise while the calcaneus anchors the hindfoot, with the nerve lying in the subcutaneous plane where it becomes an accessible landmark. This lateral view matters because it matches how the sural nerve is encountered in clinic and the operating room, where its relationship to the small saphenous vein and the posterolateral border of the Achilles tendon guides safe dissection. It is also the nerve most commonly harvested for autologous nerve grafting, and its superficial course explains why it is vulnerable during lateral ankle arthroscopy portals, percutaneous Achilles tendon repair, and posterolateral approaches to the calcaneus. A common pain generator. Entrapment and neuroma can follow ankle sprains or iatrogenic injury, producing sensory loss or dysesthesia over the lateral foot and fifth toe.
