Description

Running along the posterolateral thigh, the sciatic nerve descends deep to the biceps femoris toward the popliteal fossa, where it divides into the tibial nerve and the common fibular (peroneal) nerve near the level of the knee. From the lateral leg, the common fibular nerve courses around the fibular neck, then continues as the superficial and deep fibular nerves toward the dorsum of the foot, while the tibial nerve remains more posterior, passing behind the knee and toward the tarsal tunnel. A sural nerve track is included along the posterolateral calf toward the lateral malleolus, with its relationship to the fibula, tibia, patella, and the tarsals and metatarsals clearly established. Veins and arteries run in parallel corridors with the nerves, reinforcing the neurovascular bundles in the leg. A lateral position is the clearest way to teach the high risk segment of the common fibular nerve as it wraps subcutaneously around the fibular head, a frequent site of compression from prolonged leg crossing, tight casts, or traction injury and a classic cause of foot drop. Sural nerve anatomy also matters in day-to-day practice, both for sensory symptoms along the lateral foot and for planning nerve biopsy or harvest for grafting, where an accurate surface course helps avoid iatrogenic numbness. Landmarks stay honest here, fibular neck, lateral malleolus, and the posterior compartment exit points.
