Description
Arising from the lumbosacral plexus, the sciatic (ischiadic) nerve exits the pelvis through the greater sciatic foramen and passes inferior to piriformis before entering the deep gluteal region. From a posterior perspective in an adult male, it sits deep to gluteus maximus and posterior to the hip joint, then descends in the posterior thigh along the medial border of biceps femoris and lateral to semitendinosus and semimembranosus. Proximal relationships to the ischial tuberosity and the posterior aspect of the femur frame its course, and the tibial component is appreciable as the larger medial division within the common sciatic trunk. The caliber is unmistakable. This posterior angle matters because it matches how clinicians think about sciatica and about iatrogenic injury during gluteal injections and posterior hip approaches. The nerve’s path beneath gluteus maximus and its proximity to the short external rotators (including piriformis) make it the structure at risk in deep gluteal syndrome and in piriformis-related entrapment patterns. It also orients the viewer for nerve blocks and for understanding why posterior hip dislocation, acetabular fractures, or hematoma in the deep gluteal space can produce tibial-predominant deficits, such as weakness of plantarflexion with altered sensation over the sole.
