Description
Arising from the sciatic nerve in the posterior leg, the tibial nerve descends in the deep posterior compartment and approaches the ankle posterior to the medial malleolus, where it lies close to the posterior tibial artery and accompanying veins. At the level of the tarsal tunnel, its course is shown as it passes deep to the flexor retinaculum and divides into the medial plantar nerve and lateral plantar nerve, continuing into the plantar aspect of the foot. Framing the neurovascular bundle, the distal tibia and fibula form the ankle mortise around the talus, with the calcaneus partially visible inferiorly. Spatially, the nerve sits posteromedial to the ankle joint, then shifts distally into the plantar compartment. A posterior perspective at the ankle is the one you want when teaching or planning around tarsal tunnel syndrome, where the tibial nerve or its plantar branches can be compressed beneath the flexor retinaculum. The branching pattern matters for targeted decompression and for correlating plantar sensory loss with entrapment site, since the medial plantar nerve often tracks toward the abductor hallucis while the lateral plantar nerve courses obliquely toward the lateral sole. This relationship to the posterior tibial vessels also anchors safe dissection and helps explain why vascular pulsation can guide nerve localization. Use this plate in lower-limb anatomy teaching, podiatry and orthopaedic surgery materials on hindfoot approaches, and clinical education on nerve blocks at the ankle (posterior tibial nerve block) and postoperative neuropathic pain patterns.
