Description
Running along the posteromedial leg, the tibial nerve is traced as the larger terminal branch of the sciatic nerve after its bifurcation in the popliteal fossa. From this medial perspective, it descends deep to the gastrocnemius and soleus within the posterior compartment, accompanying the posterior tibial vessels as it approaches the tarsal tunnel posterior to the medial malleolus. Distally, the nerve divides into the medial and lateral plantar nerves to supply the plantar foot, with calcaneal branches coursing toward the heel pad. Bony landmarks, including tibia, fibula, talus, calcaneus, metatarsals, and phalanges, anchor the neurovascular relationships. Medial orientation matters because it matches how clinicians palpate and decompress the tarsal tunnel, where the tibial nerve can be compressed beneath the flexor retinaculum between the medial malleolus and calcaneus, producing plantar paresthesia and heel pain that can mimic plantar fasciitis.
