Description
Running superficially along the posterolateral calf, the sural nerve is traced from its formation by the medial sural cutaneous nerve (from the tibial nerve) with a communicating branch from the common fibular (peroneal) nerve, then descending adjacent to the small saphenous vein toward the lateral malleolus. Proximally, the tibial and common fibular nerves appear as terminal branches of the sciatic nerve in the posterior thigh, deep to the hamstring compartment and approaching the popliteal fossa. Inferiorly, the sural nerve courses over the posterior compartment muscles, with gastrocnemius and soleus forming the dominant muscular backdrop and the fibula lying lateral to the tibia as an osseous landmark. The junction site varies. This plate makes it easy to track. Posterior mapping of the sural nerve matters for nerve conduction studies and for harvesting the sural nerve as a donor graft, where iatrogenic injury can occur if its relationship to the small saphenous vein is not appreciated. Entrapment and traction neuropathy often occur where the nerve pierces the deep fascia in the distal calf, producing posterolateral calf pain and sensory loss along the lateral foot, a distribution that can be confused with S1 radiculopathy. Clear depiction of the tibial versus fibular contributions also supports teaching of common anatomic variants that change the level of union and the nerve’s lateral shift toward the ankle. Ideal use includes gross anatomy and neuroanatomy lab manuals, anesthesiology references illustrating ultrasound-guided sural nerve block at the distal calf or ankle, and surgical texts discussing posterolateral approaches around the lateral malleolus and Achilles region where the nerve is at risk.
