Description

Emerging in the posterior calf as a sensory nerve, the sural nerve is traced as it descends superficial to the gastrocnemius muscle belly and trends inferolaterally toward the posterolateral ankle. Proximally, its roots are implied at the junction where a sural communicating branch from the common fibular (peroneal) nerve joins contributions from the tibial nerve, before the unified trunk courses alongside the small saphenous vein. Superiorly the nerve lies near the popliteal region, while distally it approaches the lateral malleolus to supply the lateral foot. Orientation is posterior, with medial and lateral relationships in the calf clearly readable. Posterior visualization matters because the sural nerve is frequently encountered in both iatrogenic injury patterns and planned harvest. It is at risk during posterior calf incisions, Achilles tendon procedures, endoscopic gastrocnemius recession, and lateral ankle approaches, where its close relationship to the small saphenous vein and superficial fascia can mislead dissection. Nerve conduction studies also target this nerve, and an accurate anatomic course helps explain variable sensory loss over the posterolateral leg and lateral border of the foot after trauma or entrapment.
