Description
Arising in the posterior compartment of the leg, the sural nerve is traced from its origin as a union between the medial sural cutaneous nerve (from the tibial nerve) and the lateral sural cutaneous nerve, typically joined by a peroneal communicating branch from the common fibular nerve. From the popliteal fossa it descends superficial to the gastrocnemius, then courses inferolaterally along the posterior calf, closely related to the small saphenous vein. Distally, it passes posterior to the lateral malleolus and continues as a lateral dorsal cutaneous nerve supplying the posterolateral leg and lateral foot. A superficial sensory nerve. Posterior orientation matters because it clarifies the sural nerve’s relationship to common entrapment and iatrogenic injury zones: the mid-calf where it becomes subcutaneous and the retromalleolar region where it lies near the lateral incision corridor for ankle and hindfoot surgery. This is also the route used for sural nerve biopsy and for harvesting the nerve as a graft in peripheral nerve reconstruction, where consistent identification relative to the gastrocnemius heads and the small saphenous vein reduces morbidity. The proximal “union of two communicating branches” is a well-known source of anatomic variation, a teaching point that explains why sensory deficits after posterolateral calf trauma can be patchy.
