Description
Running from the sciatic nerve in the posterior thigh, the tibial nerve continues through the popliteal fossa and descends in the posterior compartment of the leg deep to the gastrocnemius and soleus. From this posterior perspective, the nerve tracks inferiorly along the tibia and fibula, positioned near the deep posterior muscles as it approaches the medial side of the ankle on its course toward the tarsal tunnel. Proximal skeletal landmarks such as the femur, patella, tibia, and fibula frame the pathway, with venous channels shown in parallel to the neurovascular route. Orientation is clear. Clinically, this is the nerve that matters when plantarflexion strength, plantar sensation, or intrinsic foot function is in question, because its downstream branches include the medial and lateral plantar nerves as well as calcaneal branches. The posterior calf segment is also where the tibial nerve stays close to the posterior tibial vessels, a relationship that guides deep posterior compartment dissection and helps explain why swelling, hematoma, or entrapment near the tarsal tunnel can produce burning plantar pain and toe flexor weakness. Nerve continuity from pelvis to ankle is a clean teaching line for distinguishing tibial neuropathy from S1 radiculopathy or more proximal sciatic injury.
