Description
Seen from the lateral aspect of the male lower limb, the sural nerve courses superficially along the posterolateral calf, tracking near the lateral border of the gastrocnemius and descending toward the lateral malleolus. Proximally, the sciatic nerve is implied as it divides into the tibial nerve and common fibular (peroneal) nerve, with the sural nerve arising from their contributing branches and then accompanying the small saphenous vein in the subcutaneous tissue. Deep to the nerve, the fibula defines the lateral leg, while the tibia lies more medial, with the knee (patella, femoral condyles, tibial plateau) and ankle region anchoring the proximal to distal orientation. Lateral presentation matters because the sural nerve is a common site for diagnostic nerve conduction studies and a frequent donor for autologous nerve grafting, and its superficial path makes it vulnerable during posterolateral approaches to the ankle and hindfoot. Injury is classically iatrogenic during Achilles tendon repair, peroneal tendon surgery, lateral malleolar fixation, or endoscopic procedures near the small saphenous vein, producing numbness or dysesthesia along the lateral foot and posterolateral heel.
