Description
Obliquely oriented along the posterolateral calf into the lateral foot, the sural nerve is traced from its two usual contributors, the medial sural cutaneous nerve from the tibial nerve and the peroneal (fibular) communicating branch from the common fibular nerve. After formation, the trunk descends superficial to the deep fascia, closely paralleling the small saphenous vein, then passes posterior to the lateral malleolus to continue as the lateral dorsal cutaneous nerve across the dorsolateral foot. Bony landmarks of the foot are rendered in detail, including the calcaneus, talus, cuboid, metatarsals, and phalanges, providing positional reference as the nerve courses distally toward the fifth ray. Color separation of neural pathways emphasizes the sural distribution relative to adjacent dorsal cutaneous branches. This configuration matters in procedures that target lateral ankle and foot sensation. Sural nerve harvest for autograft and iatrogenic injury during posterolateral ankle arthroscopy, Achilles tendon repair, or lateral malleolar fixation all hinge on knowing where the nerve crosses the retromalleolar groove and how proximally its contributing branches unite.
