Description
Arising from the dorsal rami of the upper lumbar spinal nerves, the superior cluneal nerves are traced as slender cutaneous branches coursing inferolaterally across the posterior iliac crest toward the superolateral gluteal region. From a posterior perspective, the thoracolumbar spine, sacrum, and bony pelvis provide the midline and inferior landmarks, with the nerve filaments fanning laterally over the posterior superior iliac spine region. Superficial targets lie over the upper buttock, lateral to the sacral midline and superior to the gluteal fold. Clear bony reference points anchor the pathways. Superior cluneal nerve entrapment is a common but under-recognized source of low back and buttock pain, often misattributed to lumbar radiculopathy or sacroiliac joint disease. The vulnerable segment is where these nerves cross the osteofibrous tunnel at the iliac crest (classically several centimeters lateral to the posterior superior iliac spine), a site implicated in focal tenderness and pain provoked by lumbar extension. Seeing the cutaneous branches in relation to the iliac crest helps explain why a small, superficial nerve can generate symptoms that mimic deeper spine pathology. Small nerve, big complaint.
