Description
Originating from the dorsal rami of the upper lumbar spinal nerves, the superior cluneal nerves course inferolaterally from the posterior lumbar region toward the iliac crest, then descend to supply the skin over the superolateral buttock. Their elongated cutaneous branches are shown crossing the posterior iliac crest near the posterior superior iliac spine, with segmental roots emerging from the intervertebral foramina between adjacent lumbar vertebrae. Surrounding landmarks include the thoracolumbar junction with rib-bearing thoracic vertebrae superiorly, paired scapulae laterally in the upper field, and the bony pelvis inferiorly, which frames the transition into the gluteal region. Color coding distinguishes the spinal nerve roots and their peripheral continuation. This anatomy matters because the superior cluneal nerves commonly become entrapped where they traverse an osteofibrous tunnel at the iliac crest, often piercing the thoracolumbar fascia near its attachment, producing superior cluneal nerve entrapment syndrome. Pain localizes to the posterior iliac crest and radiates into the buttock, a pattern frequently misread as lumbar radiculopathy or sacroiliac joint pathology when the cutaneous distribution is not appreciated.
