Description
Seen from a posterior perspective, the proximal forearm is arranged with the ulna positioned medially and the radius laterally, continuing distally to the carpal bones, metacarpals, and phalanges of the hand. Over the posterolateral elbow, the anconeus occupies the interval between the lateral epicondyle region and the proximal ulna, while the supinator lies deeper, wrapping from the lateral epicondyle and adjacent ulna around the proximal third of the radius. The supinator’s fibers spiral anteriorly and laterally to insert along the lateral, posterior, and anterior surfaces of the proximal radius, placing it in close relationship to the radial head and proximal radioulnar joint. Posterior views are where the supinator’s depth can be taught without losing the bony landmarks that orient clinicians during examination and surgery. The deep branch of the radial nerve (posterior interosseous nerve) passes through the supinator, most often compressed beneath the fibrous proximal edge (Arcade of Frohse), producing motor weakness in finger and thumb extension with minimal sensory findings. This anatomy also underpins lateral elbow pain patterns that mimic lateral epicondylitis, and it matters during exposure for radial head fracture fixation or proximal radioulnar procedures where nerve protection and muscle-splitting planes determine outcomes. Small muscle, big consequences.
