Description
Running along the proximal lateral forearm, the supinator wraps from the lateral epicondyle of the humerus and the supinator crest of the ulna to the proximal third of the radius, forming a muscular sleeve around the radial neck. Superficial to it, the extensor carpi radialis longus and brevis and the common extensor mass occupy a more posterior and lateral plane, while the brachialis remains anterior at the distal humerus, approaching its ulnar insertion. The radius lies lateral to the ulna in anatomical position, and the elbow joint landmarks, including the radial head and capitulum, orient the viewer to the proximal radioulnar relationship. Supination is the point. With the elbow extended, the supinator becomes the prime rotator of the radius, and its deep surface relationship to the deep branch of the radial nerve is the teaching takeaway in a lateral dissection-style perspective. The posterior interosseous nerve passes between the superficial and deep heads of supinator at the arcade of Frohse, a common entrapment site producing finger and thumb extension weakness without the sensory loss typical of more proximal radial neuropathy. Orthopedic and plastic surgery teams use this anatomy when planning approaches to the radial head, proximal radius, and lateral elbow, where staying in the interval between extensor carpi radialis brevis and extensor digitorum helps protect the nerve as it disappears beneath supinator.
