Seen from the posterior trunk under the skin in an adult male, the levatores costarum appear as short, obliquely oriented muscle slips spanning from the transverse processes of the lower cervical and upper thoracic vertebrae to the ribs below. Each bundle courses inferolaterally from the vertebral column toward the rib angles, lying deep to the erector spinae mass and adjacent to the costotransverse joints. Spinous processes form the midline landmark, while the ribs arc laterally to frame where the muscle fibers insert near the costal tubercles and angles.
