Description
Running obliquely from the thoracic transverse processes to the ribs, the levatores costarum appear as short, segmented slips deep to the superficial back musculature and lateral to the thoracic spinous processes. Each fascicle arises near the transverse process and descends inferolaterally to insert on the rib immediately below, with some longer slips spanning to the second rib below, forming a subtle ladder-like pattern along the posterior thoracic wall. Medially, the slips sit adjacent to the rotatores and multifidus of the transversospinalis group, while laterally they approach the rib angles and blend into the intercostal space. Overlying context typically includes trapezius and latissimus dorsi rendered muted so the deep elevator slips can be read against the thoracic vertebrae and ribs.
