Description
Posterior lumbar elements from L1 through L5 are arranged in a vertical column, with broad laminae meeting in the midline to form short, hatchet-like spinous processes that project posteriorly and sit close to one another. Paired transverse processes extend laterally from each vertebra, while the superior and inferior articular processes stack to create the zygapophysial (facet) joints just lateral to the spinous processes. Between adjacent vertebrae, the intervertebral discs and posterior annulus lie anterior to the vertebral arches, helping define the posterior margin of the vertebral canal. Thick bone. Big loads. A posterior perspective on the lumbar vertebrae is the cleanest way to teach how posterior element anatomy relates to low back pain generators and common procedures. Facet arthropathy at L4 to L5 and L5 to S1, along with hypertrophy of the facet joints and ligamentum flavum (deep to the laminae), contributes to central canal or lateral recess stenosis, and the relationship of spinous processes, laminae, and interlaminar spaces underpins midline approaches for lumbar puncture, epidural access, and laminectomy. Orientation matters, because the lumbar spinous processes project more horizontally than thoracic levels, changing palpation landmarks and the working corridor for posterior decompression. Use this artwork in gross anatomy and musculoskeletal courses to anchor vertebral level identification, in radiology teaching files to correlate with axial CT and sagittal MRI of posterior elements, or in spine surgery and pain medicine materials explaining facet blocks, medial branch radiofrequency ablation, and decompressive techniques.
