Description
Posteriorly, the right and left lungs occupy either side of the thoracic vertebral column, with the apices extending superiorly toward the root of the neck and the bases resting on the diaphragm. The posterior surfaces relate medially to the paravertebral gutter and laterally to the ribs, while the oblique fissures descend inferoanteriorly to separate superior and inferior lobes on both sides; on the right, a middle lobe lies anterolateral and is largely hidden from a true posterior perspective. At each hilum, the main bronchus and pulmonary vessels enter on the mediastinal surface, then branch distally into lobar and segmental bronchi that ultimately supply alveoli. A posterior lung view matters because the distribution of disease often respects lobar and segmental anatomy as projected on the back, and bedside examination targets these same regions. Percussion and auscultation are performed posteriorly to assess lower-lobe pneumonia, dependent atelectasis, or pleural effusion, which commonly accumulate in the costodiaphragmatic recess and blunt the posterior basal breath sounds. It is also the clearest teaching angle for relating the oblique fissure to surface anatomy when planning posteroanterior chest radiograph interpretation or thoracentesis, where needle placement must avoid the inferior border neurovascular bundle. Use this asset in gross anatomy and respiratory modules to anchor posterior surface landmarks, fissures, and lobar territories, or in clinical skills and pulmonary medicine materials that correlate posterior lung fields with physical exam and common consolidation patterns.
