Description
Oriented from a posterior perspective, the pancreas spans transversely across the upper abdomen with its head seated in the C loop of the duodenum on the patient’s right and the tapering tail extending leftward toward the splenic hilum. The slender neck bridges the head and body, lying anterior to the confluence of the superior mesenteric and splenic veins that form the portal vein. Along the posterior surface, the splenic artery typically courses superior to the gland while the splenic vein runs posterior to the body and tail, and the posterior aspect of the stomach is implied anterior-superior to the pancreas. Green vascular branching emphasizes the peripancreatic arterial and venous relationships. Posterior anatomy is where pancreatic surgery gets decided. Dissection around the neck and uncinate process risks injury to the superior mesenteric vessels and portal vein, and a clear understanding of these planes underpins pancreaticoduodenectomy and venous resection strategies. This view also maps directly to the typical pain pattern and imaging findings in pancreatitis, where inflammatory collections often track in the lesser sac and along the peripancreatic fascial planes, and to pancreatic adenocarcinoma of the body or tail, which can invade the splenic vessels or cause left-sided portal hypertension from splenic vein thrombosis.
