Description
Positioned on the lateral cerebral surface, the posterior ramus of the sulcus lateralis (posterior limb of the lateral fissure, Sylvian fissure) sweeps posteriorly and slightly superiorly as it separates the frontal and parietal opercula above from the superior temporal gyrus below. Cortical gyri and secondary sulci surrounding the fissure are rendered with enough relief to appreciate the opercular overhang and the depth of the groove. The left and right cerebral hemispheres remain distinct, with the cerebellum visible inferior and posterior to the occipital poles. A clear topographic landmark. Clinically, the posterior ramus matters because it frames perisylvian cortex, the territory most often discussed in aphasia syndromes and in stroke localization involving the middle cerebral artery.
