Description
Seen from an anterior perspective, the cerebral hemispheres are separated by the longitudinal fissure, with the frontal poles positioned superiorly and the basal surfaces tapering inferiorly toward the brainstem. Along each lateral convexity, the sulcus lateralis (lateral fissure of Sylvius) courses obliquely, forming a deep cleft between the temporal lobe inferiorly and the frontal and parietal opercula superiorly. Cortical gyri and sulci are rendered with enough relief to appreciate the gyral lips bordering the fissure. Blue markers indicate the hippocampi bilaterally, placed deep and medial within the temporal lobes relative to the lateral fissure. For teaching cortical topography, an anterior angle on the lateral fissure is a clean way to orient the opercular anatomy that overlies the insula, and to relate surface landmarks to language and sensorimotor regions. This is the fissure surgeons use as a corridor in the transsylvian approach for middle cerebral artery aneurysms and basal cisternal exposure, where careful sylvian fissure splitting limits retraction injury to the frontal and temporal opercula. Short and deep. The labeled hippocampus also supports discussion of temporal lobe epilepsy, where mesial temporal sclerosis and planned anterior temporal lobectomy are understood in relation to the temporal lobe that sits inferior to the sulcus lateralis.
