Description
Seen from the front, the left and right cerebral hemispheres frame the interhemispheric fissure, with the frontal poles superior and anterior to the visible brainstem. The lateral sulcus (sulcus lateralis, Sylvian fissure) begins on each hemisphere’s inferolateral surface and runs posteriorly, and its anterior ramus projects forward into the inferior frontal gyrus. Nearby cortical landmarks you would expect in this orientation include the precentral gyrus just anterior to the central sulcus and the adjacent postcentral gyrus posterior to it, while the temporal lobe sits inferior to the lateral sulcus. Attention to the anterior ramus of the lateral sulcus matters because it helps subdivide the inferior frontal gyrus into pars orbitalis, pars triangularis, and pars opercularis, the surface anatomy most clinicians reference when localizing Broca area in the dominant hemisphere. That relationship turns practical during preoperative planning for frontal opercular and insular lesions, where the surgeon follows the Sylvian fissure and its rami to orient the corticotomy and avoid language and motor cortex. It is also a reliable teaching landmark when comparing gross anatomy with axial MRI or CT, where the Sylvian fissure remains one of the earliest and most consistent sulcal identifiers.
