Description
Centered deep in the gluteal region, the quadratus femoris spans from the lateral border of the ischial tuberosity to the quadrate tubercle on the intertrochanteric crest of the proximal femur. It sits inferior to the tendon of obturator externus and posterior to the hip joint capsule, forming a broad, rectangular band that runs laterally toward the femur. Superiorly, the inferior gemellus and obturator internus tendon complex would lie just above it, while the proximal hamstring origin remains medial and inferior at the ischial tuberosity. Short, thick, and distinct. Posterior exposure of quadratus femoris matters because it is a consistent landmark when teaching the deep short external rotators and their relationships to the sciatic nerve and the medial circumflex femoral artery as these structures traverse the deep gluteal space. The muscle is implicated in ischiofemoral impingement, where narrowing between the ischial tuberosity and the lesser trochanter can produce edema or tearing at the quadratus femoris, a pattern often discussed alongside hip pain and snapping phenomena. This posterior anatomy also frames the internervous plane considerations during posterior hip approaches and helps clarify why lesions near the obturator externus tendon can be confused with adjacent rotator pathology.
