Description
Running along the posterior surface of the pancreas, the splenic vein courses from the splenic hilum (hilum of the lien) toward the midline, where it meets the superior mesenteric vein at the pancreatic neck to form the portal vein. The spleen sits in the left upper quadrant, lateral and slightly superior to the pancreatic tail, with the venous tributaries entering and leaving at the visceral surface beneath the splenic capsule. Short gastric and left gastro-omental venous channels may be implied near the hilum, while smaller pancreatic veins drain inferiorly and medially into the splenic vein along its path. Orientation matters. Posterior emphasis is useful because the splenic vein is frequently obscured from an anterior teaching view yet becomes a key landmark in pancreatic surgery and in interpreting cross-sectional imaging. Pancreatitis and pancreatic carcinoma can narrow or thrombose the splenic vein, producing left-sided (sinistral) portal hypertension with isolated gastric varices while hepatic function remains normal, a pattern that hinges on recognizing the venous confluence behind the pancreas. The relationship to the duodenum and pancreatic head also frames the nearby portal triad region and helps explain why inflammation or operative traction around the neck of the pancreas risks portal venous bleeding.
