Description
Arising from the splenic hilum and running medially along the posterior aspect of the pancreas, the splenic vein is presented from an anterior perspective, so the gland’s body and tail sit superficially while the vessel tracks deep to them. Laterally, the vein collects tributaries emerging from the splenic capsule and hilum, then courses toward the midline to meet venous drainage from the pancreatic parenchyma. The spleen occupies the viewer’s right side with its characteristic purple tone, while the duodenum forms a C shaped loop around the pancreatic head, marking the transition from foregut to midgut anatomy. Blue vascular channels connect spleen and pancreas, with the splenic vein running straighter than its arterial counterpart. Anterior depictions of the splenic vein matter because the structure is easy to underestimate clinically, it lies protected behind the pancreas yet becomes a major surgical and endoscopic landmark once the lesser sac is entered. Pancreatitis and pancreatic body or tail carcinoma can narrow or thrombose the splenic vein, producing left sided (sinistral) portal hypertension with isolated gastric varices while hepatic function remains normal. Surgeons also track this vessel during distal pancreatectomy and splenectomy, where loss of the proper plane can lead to brisk retroperitoneal bleeding that is hard to control. Small structure, big consequences.
