Description
Centered within the cranial base, the sphenoid bone is isolated in blue, with its body in the midline and the greater wings spreading laterally to meet the temporal and frontal regions of the skull. Anteriorly, the lesser wings frame the posterior margin of the anterior cranial fossa, while the pterygoid processes descend inferiorly toward the choanae and hard palate. Along the greater wing, the superior orbital fissure lies anterosuperior to the foramen rotundum and foramen ovale, and the foramen spinosum sits posterolateral to ovale near the spine of the sphenoid. This is a map of openings, not just bone. Surgical orientation at the skull base often begins with the sphenoid because its foramina organize the exit points of the trigeminal divisions and the course of the middle meningeal artery. The foramen ovale transmits V3 and is the target for percutaneous trigeminal rhizotomy and balloon compression in trigeminal neuralgia, while the foramen spinosum marks entry of the middle meningeal artery, the vessel torn in many epidural hematomas after temporal trauma. The sphenoid body also underlies the sella turcica and sphenoidal sinus, anchoring discussions of transsphenoidal routes to the pituitary and the venous relationships that include the cavernous sinus and basilar venous plexus. Use this superior cranial base perspective for gross anatomy and neuroanatomy teaching when students must translate a dry skull into pathways for CN III to VI, V2 and V3, and the meningeal vessels.
