Description

Obliquely viewed within a calvaria-opened male cranium, the sphenoid bone is isolated in blue at the central skull base, spanning the midline between the orbital apices and the upper nasopharynx. The greater wings flare laterally to form the posterior wall of the orbit and contribute to the middle cranial fossa, while the lesser wings project anterosuperiorly toward the frontal bone. Inferiorly, the pterygoid processes descend behind the maxilla, and posteriorly the sphenoid body abuts the basilar occipital bone at the clivus; adjacent landmarks in this angle commonly include the optic canals, superior orbital fissures, and the region of the sella turcica over the sphenoid sinus. That central position explains why the sphenoid anchors so much neurovascular anatomy and why this oblique, opened-cranium view is favored for teaching corridors. Transsphenoidal surgery to the pituitary targets the sphenoid sinus and sellar floor, while fractures through the greater wing can involve the superior orbital fissure with ophthalmoplegia or extend to the foramen rotundum and ovale affecting V2 and V3. Venous anatomy matters here as well, since the basilar venous plexus on the clivus and its connections to the cavernous sinus sit immediately posterior to the sphenoid body, a pathway relevant to cavernous sinus thrombosis and to planning skull base approaches.
