Description
Anteriorly, the falciform ligament appears as a sickle-shaped peritoneal fold descending from the inferior surface of the diaphragm to the anterosuperior (diaphragmatic) surface of the liver. Its superior attachment aligns with the anterior abdominal wall and diaphragmatic peritoneum, while its inferior margin approaches the umbilical fissure on the visceral surface, where it continues as the round ligament of the liver (ligamentum teres hepatis). Right and left lobes of the liver lie lateral to the ligament, which marks the functional midline near the plane of the middle hepatic vein. A clean midline landmark. This anterior perspective matters in upper abdominal surgery because the falciform ligament is commonly divided for exposure during open and laparoscopic approaches to the liver, diaphragm, and suprahepatic inferior vena cava. Surgeons also track it as a guide to the left portal structures, and its free edge can conceal recanalization of the paraumbilical veins in portal hypertension (caput medusae correlation), a finding that changes dissection strategy to avoid troublesome bleeding. Radiologists recognize its fat and peritoneal reflections on CT as the falciform ligament sign and in the spread of intraperitoneal fluid. Use this artwork in gross anatomy teaching on peritoneal reflections and hepatic surface anatomy, or in hepatobiliary and transplant texts illustrating mobilization maneuvers and operative landmarks from an anterior approach.
