Description
Running deep to the sternocleidomastoid muscle, the internal jugular vein (IJV) descends from the jugular foramen toward the root of the neck, positioned lateral to the common carotid artery within the carotid sheath. Superiorly, the vein lies just inferior to the angle of the mandible and adjacent to the parotid region, then continues inferiorly to join the subclavian vein behind the medial end of the clavicle to form the brachiocephalic vein. Smaller tributaries expected in this anterior neck presentation include the facial, lingual, and superior thyroid veins entering the IJV along its anteromedial margin, with the external jugular vein coursing more superficial and lateral across the sternocleidomastoid. Depth matters here. An anterior rendering of the IJV is the working view for central venous access, where the target lumen sits lateral to the carotid pulse and deep to the platysma and investing fascia, often approached at the apex between the sternal and clavicular heads of sternocleidomastoid. That spatial relationship explains two classic complications: inadvertent carotid arterial puncture when the needle drifts medially, and pneumothorax when a low approach tracks toward the lung apex. The carotid sheath corridor, plus expected anatomic variation in IJV caliber and overlap with the carotid, makes this a teaching image for safe line placement and for interpreting bedside ultrasound landmarks.
