Description
Arising at the jugular foramen as the continuation of the sigmoid sinus, the internal jugular vein descends deep in the anterior neck within the carotid sheath, lateral to the common carotid artery and anterolateral to the vagus nerve. Superiorly, the vein lies inferior to the skull base and posterior to the mandibular ramus, then continues toward the thoracic inlet where it joins the subclavian vein to form the brachiocephalic vein. An anterior perspective also places the vein in relation to midline airway structures, with the trachea and upper esophagus posterior and medial to the great vessels. Broad caliber. Deep course. Clinically, this anatomy underpins internal jugular venous cannulation, where operators aim the needle toward the ipsilateral nipple at the apex of the triangle between the sternal and clavicular heads of the sternocleidomastoid, targeting a vein that is lateral to the carotid pulse but can overlie it with head rotation or hypovolemia. The close packing of vein, artery, and vagus within the carotid sheath explains the main complications of line placement and neck trauma: carotid puncture, hematoma with airway compromise, and iatrogenic nerve injury. The anterior view also reinforces venous drainage patterns relevant to raised intracranial pressure, where jugular venous distension becomes a bedside sign and venous outflow obstruction can worsen cerebral edema.
