Description
Running deep to the sternocleidomastoid, the internal jugular vein descends from the jugular foramen toward the venous angle, lying lateral to the common carotid artery and anterior to the cervical vertebral column. Within the carotid sheath, the vagus nerve tracks between the vein and artery, a constant relationship that helps orient the neurovascular bundle in the upper neck. Superiorly, the vessel sits just inferior and posterior to the angle of the mandible, while the parotid and digastric region frame its proximal course. A lateral cervical view is the one most clinicians mentally reference during ultrasound-guided internal jugular venous cannulation, because the vein’s caliber and position change with head rotation and the overlying sternocleidomastoid heads. The carotid sheath anatomy also matters when teaching why hematoma, inadvertent carotid puncture, and vagal stimulation can occur during line placement, and why infection can track along fascial planes into the mediastinum. Clear depiction of adjacent muscles, including sternocleidomastoid and trapezius, supports surface landmarking and safe needle trajectory.
