Description
Running superficially over the lateral neck, the external jugular vein descends from the region of the parotid gland and angle of the mandible toward the root of the neck, where it approaches the subclavian vein deep to the clavicular area. Along its course it crosses the sternocleidomastoid muscle obliquely, remaining lateral to the internal jugular vein and the common carotid artery, which sit deeper within the carotid sheath. A lateral skull and cervical spine provide bony context, with the mandibular ramus, mastoid region, and cervical vertebrae framing the vessel’s surface anatomy. Clinically, this is the neck vein you can often see and cannulate. External jugular venous distension is a standard bedside sign in right-sided heart failure, cardiac tamponade, and tension pneumothorax, and a lateral view clarifies why the vein becomes prominent as it traverses the sternocleidomastoid rather than disappearing into deeper fascial planes like the internal jugular. Its termination near the subclavian vein also explains common procedural pitfalls, including difficult guidewire passage or extravasation when attempting external jugular access compared with ultrasound-guided internal jugular cannulation.
