Description
Prominent within the lateral cervical region, the internal jugular vein descends from the jugular foramen at the skull base to the venous angle, showing the characteristic superior bulb proximally and the inferior bulb just above its junction with the subclavian vein to form the brachiocephalic vein. The vessel tracks deep to the sternocleidomastoid, lateral to the carotid sheath contents, and anterior to the prevertebral fascia overlying the cervical vertebrae. Posteriorly, trapezius contours the nuchal region, while bony landmarks of the mandible and upper cervical spine frame the field. Orientation is clear. Bulb morphology matters in day to day procedural anatomy because the inferior jugular bulb is a common site of venous valve leaflets and caliber change, both of which influence catheter passage and guidewire behavior during central venous access. A high-riding jugular bulb or a prominent superior bulb can also explain pulsatile tinnitus and complicate skull base or otologic surgery, where venous bleeding risk rises near the temporal bone. For teaching, this is the cleanest way to connect surface anatomy (sternocleidomastoid borders) with deep venous drainage patterns.
