Description
Running superficially in the anterolateral neck, the external jugular vein descends from the region of the angle of the mandible, crossing superficial to the sternocleidomastoid toward the root of the neck. Inferiorly it approaches the clavicle and drains into the subclavian vein in the supraclavicular fossa, lateral to the expected course of the internal jugular vein within the carotid sheath. Bony landmarks, including the mandible, maxilla, orbital margins, and cervical vertebrae, frame the vessel’s trajectory in anterior perspective. Color coding distinguishes blue venous return from red arterial pathways. Surface anatomy makes the external jugular vein a frequent landmark for bedside assessment of venous filling, yet it can mislead when used as a surrogate for central venous pressure because valves and variable communications with the internal jugular vein alter column behavior. Cannulation attempts also have specific pitfalls: the vein is mobile within superficial fascia and lies near the external branch of the accessory nerve and the cervical plexus branches as they emerge at the posterior border of sternocleidomastoid. That relationship matters when planning a low neck incision, placing a central line, or interpreting venous distention caused by superior vena cava obstruction. Small structure, big consequences.
