Description
Running superficially over the anterolateral neck, the external jugular vein descends from the region posterior to the angle of the mandible and crosses superficial to the sternocleidomastoid toward its termination in the subclavian vein just superior to the clavicle. Deep to it, the internal jugular vein follows a more medial and deeper course within the carotid sheath alongside the common carotid artery, while the cervical vertebrae stack posteriorly as a rigid landmark behind the vascular column. Superiorly, the skull base frames the upper cervical vessels, and inferiorly the venous outflow converges toward the thoracic inlet. Bone and vessel align cleanly. Surface anatomy of the external jugular vein matters because it is one of the few major cervical veins that can be inspected directly for distension, making it a practical proxy for jugular venous pressure assessment in right-sided heart failure, cardiac tamponade, and tension pneumothorax. Its superficial course also explains why it is prone to laceration in penetrating trauma and why it can be selected for peripheral venous access when upper limb options are limited, while still carrying a known risk of air embolism when the vein is held open above the level of the heart.
