Description
Running superficially over the lateral neck, the external jugular vein forms near the angle of the mandible from the union of the posterior auricular vein and the posterior division of the retromandibular vein, then descends inferiorly across the sternocleidomastoid toward the root of the neck. Near its origin, the vessel lies posteroinferior to the parotid gland, with the auricle and external acoustic meatus providing clear bony and cartilaginous landmarks in the periauricular region. Deeper and more medial, the internal jugular vein and common carotid artery sit within the carotid sheath, while the trapezius defines the posterior cervical contour. Cutaneous tributaries and communicating veins are suggested along the course toward the subclavian vein. Palpability and visibility make the external jugular vein a practical surface landmark for jugular venous distension assessment, yet its anatomy explains common pitfalls. Valves near its termination, variable caliber, and tethering by the deep cervical fascia can limit reliability as a surrogate for central venous pressure, and the vein may collapse with platysma tension or head rotation. Cannulation attempts also fail for predictable reasons, the vein angulates where it pierces the investing layer of deep fascia and enters the subclavian, and it can be displaced laterally over the sternocleidomastoid, away from the safer deep target of ultrasound guided internal jugular access. Small but real complications include air embolism from negative intrathoracic pressure and injury to adjacent superficial cervical nerves.
