Description
Running superficial to the sternocleidomastoid, the external jugular vein descends obliquely from the angle of the mandible toward the root of the neck, where it pierces the deep cervical fascia to terminate near the subclavian vein in the posterior triangle. Tributaries likely include the posterior auricular vein and posterior division of the retromandibular vein superiorly, with contributions from the transverse cervical and suprascapular veins inferiorly. Deep to this plane, the carotid sheath contents sit medial, the common carotid artery and internal jugular vein tracking vertically with the vagus nerve between them, while the platysma and superficial fascia overlie the venous course. Regional lymph nodes and lymphatic channels accompany the superficial vessels along the anterolateral neck and lower face. A clear surface landmark. This morphology matters because the external jugular vein is the most accessible large superficial vein in the neck and its valve pattern and fascial perforation at the clavicular level influence venous distension and cannulation success. Clinicians rely on external jugular venous distension as a bedside marker in right heart failure, cardiac tamponade, and tension pneumothorax, yet isolated external jugular prominence can also reflect proximal obstruction or thrombosis. Surgeons and proceduralists also track its relationship to the great auricular nerve and the superficial cervical plexus as they emerge at the posterior border of the sternocleidomastoid, a common site of traction or iatrogenic injury during cervical incisions.
