Description
Posteriorly oriented neck anatomy centers on the internal jugular vein as it descends from the jugular foramen toward the thoracic inlet, positioned lateral to the common carotid artery within the carotid sheath. Deep to the sternocleidomastoid, the vein tracks anterolateral to the cervical vertebral column and prevertebral muscles, with the scalene region and trapezius forming the lateral and posterior muscular boundaries of the field. Green lymphatics cluster along the vein, including the deep cervical lymph nodes arrayed superior to inferior along its course. Small tributaries converge toward the internal jugular as it approaches the junction with the subclavian vein to form the brachiocephalic vein. Posterior perspective matters because it clarifies how the carotid sheath relates to the posterior triangle and the root of the neck, where the internal jugular vein becomes clinically vulnerable during line placement and neck dissection. Catheterization targets the internal jugular, yet the close association with the carotid artery and vagus nerve explains complications such as arterial puncture, hematoma, and hoarseness from nerve irritation. Deep cervical lymph node chains shown adjacent to the vessel also map directly onto patterns of metastatic spread from head and neck squamous cell carcinoma and guide selective neck dissection levels.
