Description
Seen in lateral profile, the internal jugular vein (IJV) descends within the carotid sheath from the jugular foramen toward the thoracic inlet, running deep to the sternocleidomastoid muscle and lateral to the common carotid artery. Superiorly it lies just inferior to the angle of the mandible and deep to the parotid region, then continues inferiorly toward its confluence with the subclavian vein to form the brachiocephalic vein. Facial skin and beard on the superficial half provide surface context, while the deeper half emphasizes the vessel’s large caliber and protected course in the male neck. Depth matters here. A lateral perspective is the practical teaching angle for internal jugular venous access because it reinforces where the IJV sits relative to the sternocleidomastoid heads and the carotid artery, the relationship that guides safe needle trajectory. The carotid sheath arrangement also explains common complications: carotid puncture when the vein collapses under pressure, hematoma with airway risk in the confined cervical fascial planes, and proximity to the vagus nerve posteriorly within the sheath.
