Description
Positioned in the right upper quadrant, the gallbladder (vesica biliaris) lies inferior to the liver’s visceral surface, with its fundus projecting anteriorly and its neck directed superomedially toward the porta hepatis. From the neck, the cystic duct ascends to join the common hepatic duct, forming the common bile duct (ductus choledochus) that continues inferiorly toward the second part of the duodenum. Proximal intrahepatic biliary radicles converge into the right and left hepatic ducts, which unite as the common hepatic duct just superior to the cystic duct insertion. Ductal junctions are the visual anchor. An anterior perspective is the one surgeons mentally rehearse when establishing landmarks for cholecystectomy and when interpreting extrahepatic cholangiography. The cystic duct to common hepatic duct junction, and its proximity to the hepatoduodenal ligament, frames the anatomy at risk during misidentification of Calot’s triangle and resultant common bile duct injury. This orientation also supports teaching the obstruction patterns of gallstones: a stone in the cystic duct often causes biliary colic and acute cholecystitis, while choledocholithiasis in the distal common bile duct predisposes to obstructive jaundice and ascending cholangitis. Use this artwork in gross anatomy and GI blocks to reinforce the continuity from intrahepatic ducts to the common bile duct, and in operative anatomy modules that emphasize safe duct identification before clipping and division.
