Description

Framed from the anterior aspect of a male knee, the distal femur sits superior to the proximal tibia and fibula, with the femoral condyles opposed to the tibial plateau across the tibiofemoral joint line. Anteriorly, the patella lies within the quadriceps tendon and tracks over the femoral trochlea, continuing inferiorly as the patellar ligament to the tibial tuberosity. Medial and lateral collateral ligament complexes flank the joint, while the iliotibial tract and biceps femoris tendon occupy the lateral side and the pes anserinus tendons approach the anteromedial proximal tibia. Colored neurovascular structures course around the joint, with the femoral vessels and saphenous nerve distribution suggested anteromedially and the common fibular (peroneal) nerve expected laterally near the fibular neck. For teaching and preoperative planning, an anterior knee view clarifies the extensor mechanism and its relationship to the femoral condyles and tibial tuberosity, the key landmarks for assessing patellofemoral maltracking and anterior knee pain. Patellar tendon harvesting for ACL reconstruction and approaches for tibial tubercle osteotomy both depend on knowing the layered anatomy superficial to the joint capsule and the proximity of infrapatellar branches of the saphenous nerve. This is where iatrogenic numbness happens.
