Description
Running centrally from the upper abdomen to the root of the neck, the thoracic duct is shown ascending just anterior to the vertebral column, receiving the intestinal trunk and paired lumbar trunks at the cisterna chyli in the posterior abdomen. Superficial and deep lymphatic vessels course superiorly across the anterior thorax toward the axilla, where clusters of axillary lymph nodes lie lateral to the pectoral region and drain the upper limb and breast territory. Along the midline and paraspinal region, additional lymphatic channels and nodal stations track the great vessels and esophageal corridor, linking abdominal and thoracic drainage pathways. Orientation from the anterior surface makes the “receiving trunks” concept easy to teach: most lymph from below the diaphragm converges on the cisterna chyli, then enters the thoracic duct, whereas the right lymphatic duct typically handles the right upper quadrant. This matters in oncology and surgery. Patterns of metastasis from abdominal viscera (for example, gastric and pancreatic primaries) and chylous complications after esophagectomy, thoracic aortic work, or retroperitoneal lymph node dissection all depend on these central conduits and their proximity to the spine and posterior mediastinum.
