Description
Running vertically on the anterior abdominal wall, the superior epigastric vein is traced as it ascends deep to rectus abdominis within the rectus sheath, closely accompanying the superior epigastric artery on the posterior surface of the muscle. Superiorly, its course leads toward the costal margin where it continues to the internal thoracic (mammary) venous system, while inferiorly it lies in continuity with the inferior epigastric venous drainage. Rectus abdominis sits medial, with the lateral abdominal wall musculature and the anterior thoracic musculature positioned lateral and superior in the same field. Surface muscle fiber direction and layered vascular pathways clarify depth relationships. Clinically, this vessel matters because it participates in a major collateral channel between the subclavian and external iliac systems via the internal thoracic and inferior epigastric veins, a pattern that becomes conspicuous in inferior vena cava obstruction and in portal hypertensive abdominal wall varices. It is also a practical landmark during anterior abdominal wall surgery, since trocar placement, rectus sheath dissection, and flap elevation can injure the epigastric vessels, leading to rectus sheath hematoma or troublesome bleeding.
