Anatomical Depiction of the Female Bladder Trigone

Description

Centered within the inferior posterior wall of the adult female urinary bladder, the vesical trigone (trigonum vesicae) appears as a smooth triangular field bounded superolaterally by the right and left ureteric orifices and inferiorly by the internal urethral orifice. In this anterior sectional profile, the bladder lumen is opened in cross-section so the trigone sits posterior to the pubic symphysis and superior to the proximal urethra, with the interureteric ridge spanning between the ureteric openings. The detrusor muscle surrounds the mucosa, while the trigonal mucosa remains comparatively fixed and less rugose than the surrounding bladder lining. Spatially, the ureters enter the bladder wall obliquely from posterolateral to anteromedial before opening into the trigone. Clear exposure of the trigone matters because it concentrates key urologic landmarks in a small area, and surgical misorientation here has consequences. During cystoscopy, transurethral resection, or placement of ureteral stents, the ureteric orifices and interureteric ridge guide safe instrument positioning, while the internal urethral orifice marks the bladder neck where continence mechanisms and iatrogenic injury risk converge. This is a common site for trigonal cystitis and interstitial cystitis related changes, and it is where vesicoureteral reflux reflects failure of the oblique intramural ureteric tunnel.
