Description

Centered on the smooth trigone (trigonum vesicae) of the adult female urinary bladder, an anterior section opens the lumen to reveal the triangular mucosal field bounded superolaterally by the right and left ureteric orifices and inferiorly by the internal urethral orifice at the bladder neck. The interureteric ridge (plica interureterica) spans between the ureteric openings, while the neck tapers inferiorly into the female urethra. Anterior relations lie toward the pubic symphysis and retropubic space, with the posterior bladder wall approaching the anterior vaginal wall and cervix in the female pelvis. The trigone matters because it behaves differently from the rest of the vesicae, remaining relatively smooth and firmly tethered as detrusor folds develop elsewhere, and its sensory innervation makes it a common locus for urgency and pain in cystitis and bladder pain syndrome. Orientation to the ureteric orifices and bladder neck guides cystoscopic assessment for ureterocele, vesicoureteral reflux risk, and iatrogenic injury during pelvic surgery, where edema or distortion around the trigone can obscure the ureteric jets.
