Description
Anteriorly oriented in the neck, the cervical portion of the trachea (trachea, pars cervicalis) descends in the midline from the cricoid cartilage to the thoracic inlet, its cartilaginous rings and interannular ligaments visible beneath the infrahyoid strap muscles. Superiorly, the cricothyroid region and cricoid arch sit just inferior to the thyroid cartilage, while the thyroid gland lobes lie anterolateral to the upper trachea with the isthmus typically crossing the second to fourth tracheal rings. Laterally, the carotid sheaths flank the airway, and the recurrent laryngeal nerves course in the tracheoesophageal grooves posterolateral to the trachea, close enough to matter even in a predominantly anterior view. Orientation from the front is the working view for airway access. Tracheostomy and cricothyrotomy planning relies on accurate appreciation of the cricoid, tracheal ring levels, and the relationship of the thyroid isthmus and anterior jugular venous system to the midline, because a low incision risks the inferior thyroid veins and a high incision risks subglottic injury at the cricoid. Midline anatomy matters. The same landmarks guide interpretation of anterior neck swelling, tracheal deviation from thyroid enlargement, and the safe corridor for emergent surgical airway when endotracheal intubation fails.
