Description
Running inferior to the larynx in anterior view, the trachea is shown as a series of C-shaped cartilaginous rings (cartilagines tracheales) separated by the annular ligaments (ligamenta anularia tracheae), the fibroelastic bands that bridge each intercartilaginous interval. Each ligament lies between adjacent rings, forming a continuous longitudinal column of pale, fibrous membrane along the anterior and anterolateral tracheal wall. Superiorly the trachea approaches the cricoid cartilage at the level of the cervicothoracic junction, while inferiorly the ring-ligament pattern continues toward the thoracic trachea. Anterior visualization of the annular ligaments matters because this is where the trachea gains its combination of stability and segmental compliance, the feature that allows neck flexion and extension without kinking the airway and permits mild caliber changes during coughing and positive-pressure ventilation. It is also the tissue plane encountered in midline airway access: tracheostomy and cricotracheal procedures typically traverse skin, strap muscles, and pretracheal fascia before reaching the inter-ring spaces, where the annular ligaments and adjacent rings guide incision placement and tube seating. Misplaced cuts can fracture a ring, predispose to tracheal stenosis, or complicate percutaneous dilatational tracheostomy by creating irregular cartilage flaps.
