Description
Anteriorly oriented, the cardiac mass is presented with the great vessels removed, leaving the exposed myocardium of both ventricles and the anatomic right atrium as they sit in the middle mediastinum. The right ventricle occupies most of the anterior surface, while the left ventricle forms the left (obtuse) margin and tapers inferiorly toward the apex. Superior and slightly rightward, the right atrium lies adjacent to the atrioventricular groove, and the interventricular sulcus marks the boundary between ventricles even when the coronary arteries have been stripped away. Removing the aorta, pulmonary trunk, superior and inferior venae cavae, and pulmonary veins forces attention onto chamber morphology and surface landmarks rather than vascular silhouettes, which is exactly where many learners struggle when moving between gross anatomy, operative reports, and imaging. This anterior-only view is the one surgeons and cardiologists mentally reference during median sternotomy and pericardiotomy, and it clarifies why the right ventricle is the chamber most at risk during anterior chest trauma or sternal re-entry. Surface sulci matter here. They guide safe dissection planes and help orient the course of the coronary circulation even when the vessels are not shown.
